Adding Charcoal

What strategy should I use for adding charcoal during a long smoke? Should I wait till 5-6 hours have passed then add another 40 briquettes? Is it ok that they will burn from the bottom to the top now since the hot coals are down low? Or should they be added a few each time I stir and refill the water. Although it seems like you would still have the same problem.
