Following the instructions (lower vents full open, upper vents open 5/16", 12 starter coals, 48 unlit in the cavity), typically how long should it take for the smokenator to come up to temperature and settle to steady state? Are we talking 15-20 mins or an hour or so?

On my first smoke I did exactly what the instructions said to do. I had problems coming up to temp at all. For my second smoke I used 16 lite coals instead and I was up to temp in about 20 minutes. I say this only to warn you 12 might not be enough.

Make sure the coals are white hot. I let mine burn for 20-30 minutes before they are put in.

When putting the lit starter coals in the cavity, are folks mixing they in with the unlit ones, or just dumping them in?

Just dump them in and make sure the water pan will fit in.
