I have only used my smokenator a couple of times but each times the charcoal grate and smokenator slid and wasn't tight against the kettle side when I put the lit coals in and trying to get the water pan in.. I noticed the tabs on my kettle which the charcoal grate is at an angle, approximately 45 degrees. Is this right or should the be closer to 90 degrees.

It happens so that my European version of the Weber seems to be like the Asian ones, and therefor lacks holes where the smokenator side tabs fits. Anyway I simply set the base in an appropriate space in the coal holding grate and the smokenator is still stable. To ensure that the smokenator doesn't flip while loading with hot coal i simply hold it to the side with a wire. Loaded full I remove it. No problems!
What seems to be your problem is that you push the coals to hard into the smokenator to fit the water supply. You will then push on the side of the coal grate, and it might slide off the tab. Happened to me the first time, but if you just don't press that hard you'll keep clear of this. And yes, my tabs are slight downward tilting as well, even though more like 10-20 degrees. 45 sounds a LOT.

Maybe I misunderstood. The photo above is of a tab that supports the Sn'ator and the cooking grate. The lower tabs that hold the coal grate, are slightly more than 90*. I'd venture a guess of say, 105*
